Onychomycosis: Current Trends In Diagnosis And Treatment

DYANNE P. WESTERBERG, DO, and MICHAEL J. VOYACK, DO, Cooper Medical School of Rowan University, Camden, New Jersey

    Am Fam Physician. 2013 Dec 1 88:762-770.

This version of the article contains supplmental content.

Patient information: See related handout on onychomycosis, written by the authors of this article.

Onychomycosis is a fungal infection of the fingernails or toenails that causes discoloration, thickening, and separation from the nail bed. Onychomycosis occurs in 10% of the general population but is more common in older adults the prevalence is 20% in those older than 60 years and 50% in those older than 70 years.1 The increased prevalence in older adults is related to peripheral vascular disease, immunologic disorders, and diabetes mellitus. The risk of onychomycosis is 1.9 to 2.8 times higher in persons with diabetes compared with the general population.2 In patients with human immunodeficiency virus infection, the prevalence ranges from 15% to 40%.3

Onychomycosis affects toenails more often than fingernails because of their slower growth, reduced blood supply, and frequent confinement in dark, moist environments. It may occur in patients with distorted nails, a history of nail trauma, genetic predisposition, hyperhidrosis, concurrent fungal infections, and psoriasis. It is also more common in smokers and in those who use occlusive footwear and shared bathing facilities.1,4

If you want to avoid making your own footwear a safe harbor for fungi, make sure you change out sweaty shoes and socks as soon as they become damp.

Picking fabrics that are breathable and moisture-wicking in the first place, as well as making sure they fit properly, can give you a big advantage in this regard as well.

Shoes should also be rotated, so that you dont wear the same pair two days in a row. This gives them a minimum of 24-plus hours to dry out before you put your feet back in. For extra protection, apply some antifungal powder to your shoes each evening.

The Womans Story Is A Good Reminder That Natural Products Arent Always Harmless

People have to be really cognizant of the fact that just because is botanical, that doesnt mean its not going to be an irritant or an allergen, Dr. Mona Gohara , associate clinical professor at the Yale University department of dermatology, previously told INSIDER. Theres kind of a false sense of security thinking, Oh, its botanical, its not going to hurt me. It might not be carcinogenic, but that doesnt mean its not going to irritate the skin.

And garlic isnt the only example.

Poison ivy is natural but it can also cause a bad skin rash, dermatologist Dr. Allison Arthur told INSIDER earlier this year. Another natural product I see being used a lot is coconut oil . I dont recommend using it on the face because it can clog pores and make acne worse.

That doesnt mean everyone will have negative reactions to natural ingredients. But its good to remember that theyre not universally better for skin.
