A team of scientists from the Institute for Research in Immunology and Cancer of the University of Montreal have identified vitamin B3 as a potential antifungal treatment.

Led by IRIC Principal Investigators Martine Raymond, Alain Verreault and Pierre Thibault, in collaboration with Alaka Mullick, from the Biotechnology Research Institute of the National Research Council Canada, the study is the subject of a recent article in Nature Medicine.

Infections by the yeast Candida albicans represent a significant public health problem and a common complication in immunodeficient individuals such as AIDS patients, cancer patients undergoing chemotherapy and recipients of organ transplants. While some treatments are available, their efficacy can be compromised by the emergence of drug-resistant strains.

Marginally Low Vitamin C Status Might Lead To Benefits Of Supplementation

It seems evident that any effects of vitamin C supplementation may be more prominent when the baseline vitamin C level is particularly low. As noted above, a profound vitamin C deficiency was associated with pneumonia in the early literature. It seems plausible that less severe vitamin C deficiency, which may be called marginal vitamin C deficiency, can also be associated with increased risk and severity of infections, although the effects may be less pronounced than those caused by scurvy.

Low vitamin C levels are not just of historical relevance. Cases of scurvy in hospitals have been described in several recent case reports . One survey estimated that about 10% of hospitalized elderly patients had scurvy . Surveys have also shown that plasma vitamin C levels below 11 µmol/L were found for 14% of males and 10% of females in the USA, 19% of males and 13% of females in India, 40% of elderly people living in institutions in the UK, 23% of children and 39% of women in Mexico, and 79%93% of men in Western Russia. Moreover, 45% of a cohort of pregnant women in rural India had plasma vitamin C levels below 4 µmol/L and the mean plasma vitamin C level fell to 10 µmol/L in a cohort of pregnant or lactating women in Gambian villages in the rainy season .

Impair The Effectiveness Of Niacin

Evidence suggests that taking vitamin C supplements may impair the bodys ability to increase high density lipoprotein cholesterol in people taking the combination drug niacin-simvastatin. This drug combines the vitamin niacin with the statin simvastatin , and people take it to treat high cholesterol.

Doctors consider HDL cholesterol the good cholesterol because it reduces the amount of harmful cholesterol in the blood.

Vitamin C was identified in the early twentieth century in the search for a substance, the deficiency of which would cause scurvy . Scurvy was associated with pneumonia in the early literature, which implies that the factor that cured scurvy might also have an effect on pneumonia.

Vitamin C May Decrease Common Cold Incidence In Special Conditions

Vitamin C halved the incidence of colds in five RCTs during which the participants were under heavy short-term physical activity . Three of the studies used marathon runners in South Africa as subjects, whereas one study used Canadian military personnel on winter exercise, and the fifth study was on schoolchildren in a skiing camp in the Swiss Alps, i.e., the Ritzel trial . Thus, three studies were conducted under conditions of a hot environment and profound physical stress and the other two were carried out under cold environments and physical stress .

Another group in which vitamin C has prevented colds is British men . Four trials found that vitamin C decreased the incidence of colds by 30%, and in another set of four trials, the proportion of men who had recurrent common cold infections during the study decreased by a mean of 46%. All these studies were carried out in the 1970s or earlier, and according to surveys, the intake of vitamin C in the United Kingdom was low when the studies were carried out, 0.03 to 0.06 g/day, and three of the U.K. trials specifically estimated that the dietary vitamin C intake was between 0.015 to 0.05 g/day . In particular, Baird administered only 0.08 g/day of vitamin C yet they observed 37% lower incidence of colds in the vitamin C group, indicating that it was the marginal deficiency and not a high dose that explained the benefit .

D3 Reduces Candida Fungal Burden But Not At High Dose

In the following experiments, we assessed whether vitamin D might influence susceptibility to Candida infection in vivo. Over a treatment range of 0.0011 g/kg, Candida-infected mice that received 0.01 or 0.1 g/kg of Calcijex had significantly decreased kidney fungal burden, compared with untreated mice . The weight loss trends mirrored that of fungal burden in the lower-dose vitamin D3treated mice . On the other hand, mice that received high dose of Calcijex had increased Candida growth. Findings in histological sections of the mouse kidneys corresponded to the CFUs obtained through cultures. There were fewer germinating yeast in mice treated with 0.01 or 0.1 µg/kg of Calcijex . In terms of overall survival , Candida-infected mice that received 3 daily doses of 1,252D3 had increased survival, compared with untreated mice . The administration of daily doses of vitamin D3 did not confer an additional survival benefit, compared with the 3-day Calcijex regimen described above at the same dose . Not unexpectedly, mice given a high dose of Calcijex showed poor survival, consistent with the increased fungal burden observed.
