Abstract

The lifetime incidence of nail involvement in psoriatic patients is estimated to be 8090%, and the nails can be affected in 10% to 55% of psoriatic patients. Psoriasis may also solely involve the nails, without any other skin findings, in which the treatment can be more challenging. Nail psoriasis may lead to considerable impairment in quality of life due to aesthetic concerns and more importantly limitations in daily activities resulting from the associated pain, which may be overlooked by the physicians. Several topical and systemic treatment modalities, as well as radiation and light systems, have been used in the treatment of nail psoriasis. In the last decade, the introduction of biologic agents and the utilization of laser systems have brought a new insight into the treatment of nail psoriasis. This paper focuses on the recent advances, as well as the conventional methods, in treating nail psoriasis in adults and children, in reference to an extensive literature search.

When Are The Different Treatments Considered

Most doctors recommend treating nail fungus with nail polish or cream if

• not much more than half of the nail is affected by the fungus,
• the base of the nail is not infected, and
• only some nails are affected.

Topical treatment is also usually recommended for children. One reason for this is that most oral medications arent suitable for children. Another reason is that children have thinner nails that grow more quickly, so its assumed that treatment with nail polish or creams is more likely to work in children than in adults. White superficial onychomycosis is also often treated with a nail polish or cream.

If several nails are infected by the fungus, or if the infection has spread out more on the affected nails, its usually necessary to take oral medication. And if the infection started at the base of the nail, its highly likely that only tablets will help.

How Does It Work

Clobetasol propionate is a type of medicine called a topical corticosteroid. It works by stopping skin cells releasing various inflammatory substances that make blood vessels widen and cause the skin to become red, swollen, itchy and painful. It reduces the swelling, redness and itching and so helps prevent scratching that can further irritate the skin.

Neomycin is an antibiotic medicine used to treat infections with bacteria. It works by causing bacteria to produce abnormal and faulty proteins. This ultimately kills the bacteria and clears up any infection.

Nystatin is an antifungal medicine that kills fungi and yeasts by causing holes to appear in their cell membranes, allowing essential constituents of the fungal cells to leak out.

The cream is more suitable for moist, weeping areas of skin, while the thicker, more greasy ointment is more suitable for dry, scaly areas of skin.

Symptoms Of Nail Psoriasis

You’ll know you’re getting nail psoriasis when you see these changes in your fingernails or toenails:

• Color. Your nails may turn white, yellow, or brown. They may also have small red or white spots underneath.

Ringworm is a common skin infection that is caused by fungus. Ringworm goes by many names. The medical terms are tinea or dermatophytosis. Other names for ringworm are based on its location on the bodyfor example, ringworm on the feet is also called athletes foot.

Steroid creams can make ringworm infections spread to cover more of the body. They also can change the appearance of ringworm, making it hard for healthcare providers to diagnose it.

If you travel internationally, develop a rash, and think it might be ringworm, be aware that strong over-the-counter steroid creams containing combinations of antifungal and antibacterial medicines can make ringworm worse and cause other health problems. How Can I Prevent Future Athlete’s Foot Infections

Since some people are simply more prone to fungal infections, they are also prone to repeated infection. Preventive measures include keeping your feet clean and dry, avoiding prolonged moist environments, using socks in airport security lines, removing shoes, and allowing the feet skin to “breathe,” avoiding walking barefoot, especially in public areas like swimming pools and gyms, avoiding contact with known infected people, and avoiding soaking and contaminated tool usage at nail salons. Weekly applications of a topical antifungal foot cream or sprinkling of antifungal foot powder into shoes may also be helpful.

It is imperative to take your own nail instruments, including nail files, to any public nail salon, unless you know the salon practices strict instrument sterilization and/or uses all disposable supplies.

Use cotton socks whenever possible. Make sure any affected family members also treat their athlete’s foot at the same time to avoid cross-infections.

Sets With Creams And A Nail Scraper

Treatment sets that contain two creams and a nail scraper can also be used for the topical treatment of nail fungus:

• One cream has urea in it, which softens the nail so it can be removed.
• The other cream contains bifonazole, which has an antifungal effect.

For this treatment, the affected toe or finger first has to be soaked in warm water for ten minutes and then dried. After that, the urea-based cream is applied to the nail, and the nail is covered with an adhesive bandage. After 24 hours, the bandage is removed and the toe or finger is held in warm water again. The softened layer of the nail is then scraped off using a spatula, the cream is applied again and the nail is covered with a new bandage. This treatment is carried out over 14 days. Once the infected part of the nail has been scraped away completely, the skin beneath is treated for another four weeks with a bifonazole cream.
